Public Safety Services are governed by general provisions of applicable Tariffs (posted to www.embarq.com/ ratesandconditions) and the following service-specific terms and conditions in the listed states, except in Nevada, Indiana, and Ohio where the services are governed by the applicable Local Terms of Service.

• EMBARQ® Automatic Location Identification Database Service Annex - applicable in Missouri, Nebraska, Tennessee, Wyoming, South Carolina, Virginia (only United Telephone Southeast, LLC)
  
  
• EMBARQ® Automatic Location Identification Database Reproduction Service Annex - applicable in Florida, Oregon, Texas, Kansas, New Jersey, Pennsylvania, Minnesota, Missouri, Nebraska, Tennessee, Wyoming, North Carolina, South Carolina, Virginia, and Washington
